Why 4 mm has become the default
The evidence behind short needles is fairly settled: for most adults, a 4 mm needle inserted at 90 degrees reaches subcutaneous fat reliably without needing a skin fold, and substantially reduces the risk of accidentally injecting into muscle. Intramuscular injection changes how quickly medication absorbs, which matters a great deal with insulin. Shorter needles are generally more comfortable too.
That said, the right length depends on your body and your technique — check with your pharmacist or diabetes educator before changing needle length, particularly if you're currently on a longer needle and using a skin fold.
Good practice
- Fresh needle each injection; reuse dulls the tip and raises infection risk
- Take the needle off the pen after injecting, rather than leaving it attached
- Rotate sites systematically — repeated injection in one spot causes fatty lumps that make absorption erratic
- Count slowly to ten before withdrawing, to ensure the full dose is delivered
- Used needles go in an approved sharps container. Bring full containers to us or any participating pharmacy for disposal
